Spontaneous Osteoncrosis of Medial Femoral Condyle : A Case Report

Abstract
Case histories of spontaneous osteonecrosis of medial femoral condyle have rarely been reported. We report here another such case. On examination, the knee joint was moderately swollen, tender over its medial side and gait disturbance, limited range of motion, there was a positive finding of Macmurray test. Radiograph showed, osteonecrosis finding that is, the radiolucent lesion surrounded by a sclerotic halo about 70 % of the medial femoral condyle. The magnetic resonance imaging scan shows an area of low-intensity signal with a T1 weighted image and an area of high-intensity signal with a T2 weighted image in the medial femoral condyle, which osteonecrosis. We used total knee arthroplasty as treatment. Intraoperative findings, cartilage flap was revealing a bed of subchondral bone and subchondral bone area showed necrotic tissues, fibrotic tissues and multiple bony cysts, twelve weeks after operation, the knee was asymptomatic and improved gait, range of motion without instability. There was no radiologic evidence of stem problem.
